Form with Ajax CAPTCHA

Here's an example form using the Ajax CAPTCHA technique described in the tutorial.

I've included one required field and one non-required field in addition to the CAPTCHA routine, to show how CAPTCHA interacts with ordinary client-side form validation.

The CAPTCHA validation is NOT case-sensitive.
